学生の皆さま

現在、大学生の心身の健康に関する調査への回答をお願いしています。東北大学学学生相談・特別支援センターでは、みなさんが心身ともに健康的な大学生活を過ごせるように、サポートしております。本調査は、その一環として実施するものです。


回答期日：2025年4月1日 ～2025年 5月20日（定期健康診断終了時まで）
対象者：全学生
回答方法：以下の方法でご回答ください。回答にかかる時間は約５分です。

▶学部新入生：「入学者の手引」に同封した調査紙に回答し、4月のオリエンテーション
時に提出してください。

▶それ以外の学生：以下のURLからご回答ください。回答時にはDCメールアドレスで
ログインしてください。

For All students

The Center for Counseling and Disability Services at Tohoku University supports you so that you can lead a mentally and physically healthy student life. As part of this, we will conduct this Questionnaire. It will take about 5 minutes.

When : April 1st, 2025 ～ May 20th, 2025（Until the end of Regular Medical Checkups）
Who: All students
How: Please answer according to the method below. It will take about 5 minutes.

▶Newly enrolled undergraduate students: Please answer the Questionnaire enclosed in the “Enrollment Guide” and submit it at the orientation in April.

▶Other Students: Please answer the online Questionnaire from the URL below. When answering please log in with your DC email address.
